Details: Provides a comprehensive range of social workservices to a variety of patients in an inpatient or partial hospitalsetting. Acts as the primary contact with external providers andparticipates in each assigned patientâ€™s treatment plan. Is theprimary individual responsible for patient discharge planning. Thisposition also assumes responsibility for all Utilization Reviewduties for their caseload and is responsible for leading regularlyscheduled therapeutic groups in the partial program.â€¢ Psychosocial Assessment: Initial psychosocial assessments are accomplished within the firstseventy-two (72) hours from admission or before. Assessments,treatment plans and goals reflect integration and analysis of historyand clinical observations. Plans are realistic, specific and goaloriented. Obstacles to plans or desirable outcome are identified.Assessment is complete through contacts with patients, families,outpatient providers, etc. Initial assessment and plans includeadequate evaluation of living situation support networks, stressorsand discharge planning.â€¢ Case Management/Utilization Review: Screens all new assigned cases within twenty-four (24) hours orone business day, in order to assess patientsâ€™ livingsituations and identify cases presenting disposition problems.Documents need for current level of care in progress notes, andactively seeks alternative levels of care as indicated by assessment.Actively identifies obstacles to achieving positive outcome, andworks with the treatment teams, supervisors and managers to resolvethe obstacles. Assumes responsibility for conducting all UtilizationReview for the patients and works with payers in determining covereddays. Coordinates discharge planning with patients, families andoutside agencies as necessary, through individual and familymeetings, telephone contacts and interagency conferences, to provideour patientsâ€™ insurance providers with information pertainingto hospital level of care. The discharge planning interactions aredocumented in progress notes. Reviews/discusses discharge planningactivities at team meetings, and documents these twice weekly attreatment plan updates. Attends multidisciplinary team meetings, andprovides input to team members regarding psychosocial problems,overall treatment objectives, and treatment and discharge planningfor each patient as reflected in written documentation.
